Our clinical staff works with patients to address various health needs, but what makes health centers unique are the support services available to our patients. The primary goal of a Community Health Worker (CHW) is to reduce barriers that patients face in accessing care. CHWs assist patients in identifying resources and services that best meet their needs. In addition, workers are out in the community educating individuals and families on services that are available and how they can access them.
